feat(vcs): approvals and groups; portable test scripts

- vcs settings approvals=N: lands wait until N people other than the
owner run vcs approve on exactly what lands; the last approval lands it
- vcs group <name> @people: project groups for vcs access (paths and
  changes); owners only
- both servers; spec, guide and the protocol notes updated, including
  POST /edits
- the suites use perl -pi instead of GNU sed -i, so they run on macOS
