feat(vcs): faster saves and status, tree claims, real-CLI agent run

- status and diff read only folders that differ; the index keeps its tree,
  isn't fsynced, isn't rewritten when nothing changed, and its racy window
  is 50 ms on nanosecond timestamps (2 s on coarse ones)
- publishing skips trees the host already has, and get saves once so the
  first land is quick: status on 20k files 114 -> 66 ms, 63 agents landing
  at once on 15k files 17 s -> 1.2 s
- someone who can't see a path can't put a tree holding it in a change
  (both servers)
- spec and guide match what's built; the agent run on the real CLI passes
  14/14 tasks (tests/agent)
