fix(sdk,ui,cli): third Sol pass — cross-install record ids, stale connection reads, preview of external Node apps

From Sol's re-check of the SDK/CLI/kit fixes (14 of 17 originals fixed;
these close the rest and what the fixes introduced):

- Integration template: deterministic issue ids now include the install.
  Record ids are global, so two workspaces syncing the same repository
  collided on the first insert, permanently.
- useConnection: a status read that started before a connect/disconnect
  no longer overwrites the newer status (per-key generation).
- The test workspace enforces the job effect ceiling like production (a
  write tool starting a destructive job passed in tests, failed live).
- External-backend apps that import Node modules now preview: built-ins
  are stubbed in the preview bundle (objects can be created; calling them
  says the code runs on your server) instead of a 500.
- A malformed accountUrl no longer makes manifest validation throw.
- Calendar follows a controlled value the parent changes (it kept showing
  the first month).
- The design guide points agents at the shipped types (dist/index.d.ts),
  not src, which the npm package does not include.

(External → destructive jobs and timestamp removal, also reported, were
fixed in the previous commit.) Kit gallery 14/14; all templates check.
